Walk-In Commercial Driver's License (CDL) Physical Exams
Commercial drivers are required to obtain and retain a certificate of health to make sure they are healthy and safe to be on the road operating heavy machinery and vehicles. These physicals need to be updated periodically.
CDL physical exams are very detailed and include not only a general physical exam, but a detailed history and additional testing. If you are required to have a drug and alcohol screening, you will first be notified by your employer.
What to bring:
- Copy of the physical exam form (you may download it from the following link), and complete sections 1 and 2: http://www.ct.gov/dmv/LIB/dmv/20/29/R-323.pdf
- Copy of medical examiners certificate: http://www.ct.gov/dmv/LIB/dmv/20/29/B-328.pdf
- Method of payment. The CDL Physical fee is $150.00.
- Completed pre-registration form (optional).
